INSTRUCTIONS: Combine all two (3) topics into one paper organized by title (see in all caps). You will be
required to present ALL of your findings via a paper presented to you instructor.
• 1. PERSONAL WELLNESS PLAN: What happens to your personal wellness goals after you finish this wellness
class? First, discuss your progress made with each dimension/each goal throughout and at the end of the
semester. Then, establish one personal goal for each of the eight wellness dimensions. This goal should be
reachable, realistic and measurable in nature. Each dimension/goal needs to be one (1) paragraph per goal
summarizing your progress and continuation plans for the future.
• 2. HEALTH APPRAISAL PLAN: How will you maintain good health and fitness levels or improve upon these
levels? You will also report your findings and interpretation with your results for the health appraisal day and
blood draw as applicable. You will list the items you need to maintain and the items you need to improve upon
per your results with a goal of how to maintain or improve. Research for interpreting your results needs to
include the textbook or web use or use of a credible, reliable resource. Each goal/reporting needs to be one
(1) paragraph per goal summarizing your progress and continuation plans for the future.
• 3. WELLNESS RESOURCES ON CAMPUS: What resources and support are on or provided through UIndy’s
campus to help you achieve and/or maintain your goals? You will also report at least two resources per each
wellness dimension you can find on or through the UIndy campus AND why you feel these are relevant
resources for YOU. Each dimension/resource needs have appropriate information provided (name of resource
or service, location, telephone #, website, hours of operation, etc.).

Life After Wellness
1. PERSONAL WELLNESS PLAN:
After this wellness class, my personal wellness goals will be clear and I will be determined to achieve all the goals. I have tried my best to progress in each dimension out of the eight dimensions. First I will start with the physical dimension. My goal was to do necessary physical activity everyday and to stay healthy. When I started with this goal, I was not so devoted. Slowly and steadily I have improved a lot, and now I am dedicated to an hour of physical activity regularly and I can see a great difference in my fitness. The second dimension I really stressed on was the social dimension. My goal was to become social and make more friends and I have succeeded in this goal and I am sure I will keep on progressing in the social.
The goal of my spiritual dimension was to gain some meaning in life. I am trying to achieve it and in due course of time I will understand the meaning of life to some extent. My goal of occupational dimension was to gain satisfaction from my work. I have been trying very hard to fulfill this goal and I believe I have accomplished to some extent. The goal of intellectual dimension I had selected was to become more creative and develop my creative abilities. I think I have acquired some creative skills and I am still developing them. The goal of financial dimension was secure my future financial situations. I hope my education, hard work and determination will help me to fulfill the goal. The goal of emotional dimension was to create satisfying relationships. Throughout the year, I have been practicing hard to make and maintain good relations. The environmental dimension helped me to choose the goal of developing a supportive environment. I think I have succeeded in creating a successful environment.
Now one personal goal for each of the eight wellness dimensions is:
