Cover Page (Include name) 2. Executive Summary (Write last to focus on key points/findings) 3. Introduction Current state, analysis and relevant background information, including a synopsis of information regarding the case. Problem statement, problem solving team and problem owner. 4. Body (RCA) • Analysis of Case • Key Issues • Potential and or Root Causes • Target Goals • Counter Measures • Has cause and effect been confirmed Include: Decision Criteria • Data Analysis (KPI Graphs and analysis) • RCA Justification/Predicted Outcome: It is imperative that all counter measures are founded upon RCA rationale and an understanding of the KPI’s related to the target and anticipated financial or changes and impact. 5. Conclusion Summarize your conclusion in clear, simple language. Do not simply reiterate the previous sections. Provide a synthesis of key findings presented in the paper to show how these converge to address the problem. Make sure you document any limitations of your case study and needs for further research. 6. References Sources; business journals, periodicals, data, interviews and textual references as well as any online research. Make sure you support your ideas with facts and figures.
